Both DS4 Windows and Input Mapper used this utility as a base for their development. I would suggest uninstalling DS4 Windows/Input Mapper (or any other wrapper you might be using) and then use the tool linked below. However, because they do not include a Direct Input profile for those games, you'd have to manually configure the inputs from within Windows in order for the controller to work correctly with Direct Input. Both the original Dark Souls and Dark Souls 2 recognize the first connected device, so that's why you get the spinning camera and mostly non-functioning inputs the game is defaulting to the Direct Input device since both games technically support Direct Input. Unfortunately Windows 10 doesn't allow the "Exclusive/Hide DS4" function to work. I'm guessing your are using Windows 10 and using DS4 Windows or Input Mapper as your Xinput wrapper. You'll see a generic wireless controller and an Xbox 360 Controller. You will see this if you go to your Devices & Printers. The initial driver for the Dual Shock recognizes it as a Direct Input Device (since Sony doesn't have an official PC driver) and then whatever Xinput wrapper you are using allows Windows to see it as an Xbox 360 Controller. If that is the case, remove it, if not, it's the first issue. It's because a Dual Shock controller is recognized as being two separate devices or you have an actual second controller connected and it is the default device.
